Please complete the following questionnaire and return to STR within 7 business days to allow for the necessary audit arrangements on behalf of Wal-Mart Stores, Inc. In the event STR does not receive this completed questionnaire within 7 business days, we will advise Wal-Mart Stores, Inc. and STR will not be held responsible for any cost incurred related to denied access due to lack of factory response. Your cooperation is greatly appreciated.

4. Is there more than one factory under the business license of the factory listed in number 3 above? If yes, please advise each factory covered under one license, address of each, and the distances from your factory address above.
